Read the QT note before a 20 mg vardenafil order

A 20 mg film-coated Levitra tablet is the ceiling this shelf puts in the title. Most US starts are 10 mg. 2.5, 5, and 10 mg stay in the body. QT caution sits on this card before any order. Congenital long QT and Class IA or III antiarrhythmics are the labelled 'avoid' set. High-fat meals cut Cmax by 18% to 50%. The melt tablet at 10 mg is a different product from this swallow lock. Mail [email protected] with the ECG story before Ingrid stamps 20 mg.

01 Section

Twenty milligrams is the ceiling this order has to survive

Twenty milligrams is the labelled maximum for film-coated vardenafil, and it is the lock this Copenhagen shelf puts in the title, the H1, and the descriptions. Most US labels still start most men at 10 mg about 60 minutes before sex, then allow a rise to 20 mg or a drop to 5 mg. 2.5 mg exists for the sharp interaction cuts. Those smaller tablets may be named here. They are not this lock.

Buy-Levitra banners like 20 mg because the number looks decisive. On this shelf, 20 mg is the last step on the film-coated ladder, not the first ask. Ingrid will not stamp 20 mg because a cart was cheaper per milligram. She stamps it after a QT look, a nitrate look, and a reason 10 mg was not enough or not the plan.

Staxyn 10 mg orally disintegrating tablet is a different product with higher systemic exposure than 10 mg film-coated Levitra. It is not a dissolve-in-the-mouth version of this 20 mg card. Splitting a 20 mg film-coated tablet does not create Staxyn. Swallowing Staxyn with water is the wrong technique for that ODT.

Write [email protected] with the last ECG and the antiarrhythmic list before a 20 mg order. The what-makes-it-different guide keeps QT, food, and the ODT split in one place. The PDE5 comparison is for choosing among the three locks, not for stacking them.

High-fat food can cut the 20 mg peak by up to half

Absorption
20 mg fasted Tmax 30-120 min (median 60). High-fat: Cmax down 18-50%. Bioavailability about 15%.
Distribution
Vss about 208 L. About 95% protein bound (parent and M1).
Metabolism
CYP3A4 predominant; CYP3A5 and CYP2C9 contribute.
Excretion
Half-life about 4-5 hours. Feces 91-95% as metabolites; urine 2-6%.

Two US food-effect studies on film-coated vardenafil showed high-fat meals reduced Cmax by 18% to 50%. Median Tmax after a 20 mg fasted dose is about 60 minutes, with a 30-minute to 2-hour range. European film-coated language also reports a 1-hour later Tmax and about 20% lower Cmax after a 57% fat meal, with AUC unchanged. A 30% fat meal left Tmax, Cmax, and AUC alone.

The US label still allows the tablet with or without food. That permission is not a claim that a burger leaves a 20 mg peak untouched. If the order is 20 mg because 10 mg felt weak, taking the 20 mg with a high-fat lunch can erase the step you just paid for.

Absolute bioavailability is about 15%. Clearance is about 56 L/h. Terminal half-life of parent and the M1 metabolite is about 4 to 5 hours. Fecal excretion of metabolites is about 91% to 95%; urine is about 2% to 6%. Vss is about 208 L. Those numbers sit behind a tablet that looks 'strong per milligram' and still misses when the plate is wrong.

European counseling also allows 25 minutes to 4 or 5 hours before sex for film-coated vardenafil. The US sentence most clinicians repeat is about 60 minutes. I use 60 minutes on a modest plate. I do not invent a faster onset for 20 mg than the Tmax range supports.

03 Section

Class IA and III stay off the 20 mg order

Rhythm list that keeps 20 mg in the drawer

  • Class IA examples on label: quinidine, procainamide - avoid vardenafil
  • Class III examples: sotalol, amiodarone - avoid vardenafil
  • Congenital long QT: avoid
  • Additive QT with another QT-active drug: treat as a real peer-pass, not trivia

Quinidine and procainamide are the Class IA names the vardenafil labels use as examples. Sotalol and amiodarone sit in the Class III example set. Men on those drugs should avoid vardenafil. That is not a 'watch the pulse and try 20 mg'. It is an avoid.

Congenital long-QT syndrome is the other labelled avoid. A family story of sudden death at a young age without an ECG in the chart is a pause, not a 20 mg cart. I would rather see a QTc from the last physical than a review-star screenshot.

Other QT-prolonging drugs can add. The postmarketing combination work is why I ask for the whole list: antiemetics, some antipsychotics, some antimicrobials. I will not print a homemade 'safe combo' table. I will fail the peer-pass when the list is long and the ask is already 20 mg.

If the rhythm drug cannot move, this lock may not belong on the shelf. The sildenafil 25 mg note and the tadalafil 10 mg note have their own problems. They do not inherit vardenafil's QT sentence. Choosing among them is the comparison, after the ECG story is honest. A 20 mg banner does not outvote a dirty rhythm list.

Headache, flush, and the rare stops on 20 mg

20 mg order card

This lock20 mg film-coated ceiling

QTAvoid congenital long QT; avoid Class IA/III

Headache and flushing are the reactions men notice first when the peak is real. Rhinitis and dyspepsia sit in the same everyday cluster for the class. A 20 mg tablet can be a louder peak than 10 mg. That is sometimes the point of the step. It is also the reason 20 mg is a poor first experiment in a man who already gets migraine or reflux from 10 mg.

Sudden vision loss and sudden hearing loss are class stops. NAION language is the same family of caution you see on Viagra. An erection past four hours needs urgent care. Anatomical deformity of the penis and the hematologic priapism risks belong on the card before the first 20 mg, not after a Friday night in the emergency department.

Back pain is less of a tadalafil-style next-day story for most vardenafil counseling, but muscle ache still happens in the class. I do not invent a vardenafil-specific percentage that I did not pull from this label pass. I do ask whether the 20 mg step was worth the head.

If 20 mg is tolerated and still weak after a modest plate and real stimulation, the problem may be the diagnosis, not the milligram. I will not add a sildenafil 25 mg 'booster'. I will ask whether the lock should move to another molecule on the comparison.

05 Section

Nitrates and riociguat still shut the 20 mg drawer

Organic nitrates remain contraindicated with vardenafil. The class effect is the same hypotension potentiation you already know from sildenafil and tadalafil. A 20 mg ceiling does not buy a home nitrate hour. Tell emergency care the last vardenafil time. Do not spray nitroglycerin in the hallway because the tablet was 'only 20'.

Adempas and other guanylate-cyclase stimulators stay in the same closed drawer. PDE5 inhibition plus that pathway is a pressure problem, not a theoretical one. A PAH cabinet and a Levitra 20 mg blister fail the peer-pass before QT is even discussed.

Alpha-blockers need a stable dose first and a cautious vardenafil start. The labelled interaction work with terazosin and tamsulosin produced standing systolic outliers and, in the terazosin-plus-vardenafil Tmax-overlap arm, an early stop. I will not stamp 20 mg as a first vardenafil tablet on a fresh alpha-blocker. A two-week tamsulosin start is not automatically 'stable'.

Do not stack 20 mg with sildenafil or tadalafil because one 'did not feel like enough'. One PDE5 product. If 20 mg is not the right molecule, change the lock. Do not add a second enzyme block. Chest pain after 20 mg is a hospital problem even if the last swallow was hours ago.

More potent per milligram, still a 5 / 10 / 20 ladder

Vardenafil is more potent milligram-for-milligram than sildenafil at PDE5. That is a pharmacology sentence, not a reason to skip 10 mg and order 20 mg because '20 is still less than Viagra 50'. The film-coated ladder is 5, 10, and 20 mg for most men, with 2.5 mg reserved for the brutal interaction cuts.

Most labelled starts are 10 mg. Twenty milligrams is the maximum recommended dose. Five milligrams is the step down when 10 mg works but the head or flush does not. Once per day is the frequency ceiling. Sexual stimulation is still required.

Ritonavir with vardenafil is a hard small-dose story: a 5 mg vardenafil dose with ritonavir 600 mg twice daily raised vardenafil AUC 49-fold and Cmax 13-fold, and stretched half-life to about 26 hours. The labelled cap is a single 2.5 mg not more often than every 72 hours with ritonavir. That man is not a 20 mg candidate.

Indinavir 800 mg three times daily with vardenafil 10 mg raised AUC 16-fold and Cmax 7-fold. The labelled cap in that setting is a single 2.5 mg in 24 hours. Strong CYP3A4 inhibitors in general pull the card down, not up. A 20 mg order in that chemistry is a failed Lift.

07 Section

Staxyn 10 ODT is not this film-coated 20 mg

Film-coated 20 mg versus the ODT. Do not split or melt one into the other.

Usual labelled start10 mg film-coatedNo - different step
Staxyn ODT10 mg only; higher exposure than 10 mg coatedNo

Staxyn 10 mg ODT is not interchangeable with vardenafil 10 mg film-coated. The ODT produces higher systemic exposure than the 10 mg coated tablet. That sentence is on the Staxyn label for a reason. Men who 'just want the melt' are asking for a different lock with a different AUC.

Twenty milligrams film-coated is this page's lock. It is not 'two Staxyn'. It is not 'Staxyn plus a leftover 10'. The ODT is placed on the tongue without liquid. Water with Staxyn changes the ODT's AUC and Tmax. None of that technique applies to an orange film-coated 20 mg tablet, which is swallowed.

Levitra film-coated tablets were 2.5, 5, 10, and 20 mg. The 20 mg tablet is the top of that row. If a listing says 20 mg ODT, treat it as a wrong product until a pharmacist shows an actual NDC. I have not shelved a 20 mg ODT on this card.

If the only reason someone wants Staxyn is dysphagia, say that in the mail. If the only reason they want 20 mg is a banner, say that too. Ingrid will pick one product. She will not convert an ODT into a 20 mg swallow by arithmetic.

08 Section

The QT tick is read before the 20 mg is shelved

Therapeutic 10 mg and supratherapeutic 80 mg film-coated vardenafil produced small QTc increases in the labelled thorough-QT work, in the same neighborhood as moxifloxacin 400 mg depending on the correction. Absolute QT did not lengthen the way a dirty QT drug does. The operational sentence on the label is still caution in congenital long QT and in men on Class IA or Class III antiarrhythmics.

A postmarketing combination study found an additive QT effect when vardenafil was paired with another QT-active drug. That is why 'a little QTc' is not a shrug on this shelf. I want the family history, the last QTc if one exists, and the full list of QT-prolonging drugs, not a thumbs-up from a sales page.

Twenty milligrams is a higher exposure than the 10 mg therapeutic arm of that QT study. I do not invent a millisecond delta for 20 mg that the table did not print. I do treat 20 mg as the ceiling that needs a cleaner ECG story than a first 10 mg in a man with a messy rhythm list.

Sildenafil had a small QTc move in the same kind of work. Tadalafil's counseling emphasis sits elsewhere. This page still leads with QT because that is what makes vardenafil's 20 mg order different on a daylight shelf. If the rhythm list is dirty, another lock may be the honest card.

Where a 20 mg film-coated six-count is quoted

Generic vardenafil 20 mg, six tablets, the labelled ceiling on this Copenhagen shelf, marked September 2026. GoodRx's Levitra table lists 20 mg x 10 at $573.78 average retail and $44.21 with a coupon. Those figures stay in this caption. Each link is that pharmacy only. Staxyn 10 mg ODT is a different fill. Bellis does not dispense. Prescription and a QT screen still come first. ZIP moves the number.

Six tablets of 20 mg is the count this shelf asks a US window to price for a short ceiling trial. I do not invent a cash figure in this sentence. Coupon boards often show 20 mg x 10. Ask the clerk to price six if that is the script. The quote card under this section keeps sourced figures in the caption and links each chain to its own pharmacy page.

Staxyn 10 mg ODT is a different fill and a different NDC. A 10 mg film-coated bottle is a different fill. If the window substitutes either because 20 mg was out, that is a lock change. It is not a favor. A 'Levitra melt 20' listing is a wrong product until a pharmacist shows an NDC that actually exists.

Bellis does not dispense. A prescription and the QT screen still come first. ZIP changes a print. Membership rules at a grocery pharmacy are that pharmacy's rules. A cheap 20 mg cart that skipped the ECG story is not a fill this shelf will endorse.

Mail [email protected] if a listing says 20 mg ODT or 'Levitra melt 20'. Dr. Ingrid Sorensen will want the imprint, not the banner. Bring the last 10 mg outcome if there was one. Twenty milligrams is a step, not a first Saturday from a filename.

10 Section

What has to be on the mail before 20 mg is stamped

Send the QT story, the Class IA or III list, the last nitrate, and whether Staxyn already sits in the cabinet. The lock is 20 mg film-coated. 2.5, 5, and 10 mg are other steps. The ODT is another product.

Peer-pass the food: a high-fat lunch can cut the peak you just stepped up to. Peer-pass ritonavir and other CYP3A4 inhibitors: those men are 2.5 mg stories, not 20 mg stories. Do not stack another PDE5 tablet if 20 mg disappoints.

Mail [email protected] with the ECG and the imprint. Ingrid shelves one ceiling. The difference guide is the next note if the only question is why this molecule carries a QT line the others do not lead with.

A 20 mg order that skipped 10 mg, skipped food, or skipped the rhythm list is not ready for a stamp. Write what was tried and what the last QTc said. Dr. Ingrid Sorensen will leave the drawer empty before she shelves a ceiling on a dirty card. If the listing was an ODT, say so in the first line so the peer-pass does not waste a day on the wrong product in hand.

Letters that reached the shelf

Answered by Dr. Ingrid Sørensen, MD · Internal medicine, endocrinology and clinical pharmacology, Copenhagen daylight shelf

Shelf mail for the 20 mg film-coated lock comes to [email protected]. I am Ingrid Sorensen. I want the QT story before I talk about a Levitra 20 mg order. Name the antiarrhythmic, the last ECG, and whether a Staxyn blister is already in the house.

Jesper Ravn #01

Most leaflets start at 10 mg. Why would you ever shelf 20 mg as the card on this page?

Dr. Ingrid Sørensen

Because 20 mg is the labelled ceiling for film-coated vardenafil, and this domain's lock is that ceiling, not the usual start. I still start most men at 10 mg in clinic. I put 20 mg in this title so an order for the top tablet has to walk past QT, food, and the ODT confusion instead of past a 'buy 20, it is stronger' banner.

The step to 20 mg is for a man who already tried 10 mg with stimulation, on a modest plate, without a prohibited rhythm drug, and still had an inadequate response. It is not a first Saturday and not a way to outrun a high-fat lunch. If 10 mg was never tried, 20 mg is the wrong Lift.

Mail [email protected] with how 10 mg actually behaved. If there was no 10 mg trial, I will send you back to that step. If 10 mg worked and the only ask is a cheaper 20 mg bottle from a listing, that is not a medical reason. Ingrid shelves a ceiling after a reason, not after a price.

Sigrid Holmgren #02

His father takes sotalol. Does that close a 20 mg vardenafil order for him too, or only if he swallows sotalol himself?

Dr. Ingrid Sørensen

The avoid is for the man who takes the Class III drug, not for a first-degree relative who takes it. Sotalol in his father's pillbox does not close his 20 mg drawer by genetics. A family history of sudden death or known long QT in the family does pause the stamp until someone has looked at his own ECG.

If he himself is on sotalol, amiodarone, quinidine, or procainamide, vardenafil is an avoid at every film-coated strength, including 20 mg. I will not bargain that into a 'small 5 mg try'. The QT sentence is not dose-optional on this shelf.

Write me his own drug list and whether he has ever had a QTc reported. If the only QT drug in the house is his father's, say that clearly so I do not fail the peer-pass on a roommate's blister. If his own list is clean, we can talk about whether 20 mg is even the right first card, which it usually is not.

Mikkel Frost #03

Can I treat Staxyn 10 as half of a 20 mg Levitra and skip the swallow tablet?

Dr. Ingrid Sørensen

No. Staxyn 10 mg ODT is not interchangeable with 10 mg film-coated Levitra, and it is certainly not half of a 20 mg film-coated tablet. The ODT gives higher systemic exposure than 10 mg coated. Two ODT tablets would not be a labelled 20 mg film-coated dose. One ODT is not a 20 mg swallow.

Technique differs too. Staxyn goes on the tongue without liquid. Water changes that product's AUC and Tmax. A film-coated 20 mg tablet is swallowed. Melting a 20 mg coated tablet in the mouth is a homemade ODT, which I will not shelf.

If swallowing is the problem, say so and we talk about whether an ODT lock belongs at all. If 20 mg film-coated is the lock you came for, you need that tablet, a QT pass, and a modest plate. Mail the product name on the box, not the word 'vardenafil' alone. Ingrid will not arithmetic one into the other.

Pernille Kruse #04

He wants the 20 mg at 2 pm after a high-fat lunch. What actually delays?

Dr. Ingrid Sørensen

Cmax can fall by 18% to 50% with a high-fat meal on the US food-effect studies. Tmax, already a median of 60 minutes fasted after 20 mg, can slide later. European film-coated data after a 57% fat meal showed about a 1-hour later Tmax and about 20% lower Cmax, with AUC holding. A 30% fat meal did not move those parameters.

So a 2 pm 20 mg after a heavy lunch can be a weaker, later peak than the 10-to-20 step assumed. If 10 mg felt weak because it followed gravy, 20 mg after gravy is a louder grocery bill, not a cleaner experiment. I would rather move the tablet before the meal or lighten the plate than raise milligrams.

Food is still permitted on the US label. I am not inventing a fasting-only rule. I am telling you why a 20 mg order can disappoint at 2 pm. Mail what he actually ate. If every afternoon is high-fat, the tadalafil 10 mg weekend note is food-independent. That is a molecule change, not a 20 mg plus dessert plan.

Oskar Lindén #05

An online listing says cheap 20 mg and ships fast. What does a Copenhagen peer-pass still need before you would shelf it?

Dr. Ingrid Sørensen

A prescription, a QT look, a nitrate and riociguat check, and a product that is actually film-coated 20 mg rather than Staxyn or a 10 mg bottle. Cheap and fast do not replace those. I also want to know whether 10 mg was ever tried on a modest plate with stimulation. A first order of 20 mg because the listing had 20 mg in the filename is not a Lift.

Ritonavir, indinavir, and other strong CYP3A4 inhibitors turn this into a 2.5 mg story with long gaps, not a 20 mg story. Alpha-blockers need stability first. I want that list in the mail, not in a chat after the parcel arrives. Bellis does not sell the tablet. The fill card on the page only points at US pharmacy pages and sourced quote captions.

Write [email protected] with the listing's strength wording, his ECG story, and the last 10 mg outcome if there was one. If the listing says ODT or 'melt', stop. If it says 20 mg film-coated and the peer-pass is dirty, I will say no in daylight. Ingrid would rather leave the drawer empty than stamp a ceiling on a bad rhythm list.

Every answer here is general teaching, not a decision made for the person who wrote in. What is right for you turns on your history, your other medicines, your kidneys and your blood pressure — a conversation for a prescriber who can see all of it at once.
