CYP3A4 handles most of this tablet in the liver. Strong inhibitors such as ketoconazole, itraconazole, or ritonavir raise exposure. For as-needed Cialis with those inhibitors, the US maximum is 10 mg, and not more often than once every 72 hours. That is a tighter lid than the usual once-a-day line.
Creatinine clearance between 30 and 50 mL/min changes the as-needed start. The label wants 5 mg not more than once per day as the start, and a maximum of 10 mg not more than once every 48 hours. Exposure roughly doubles in that renal band in the clinical pharmacology work the label cites.
Below 30 mL/min or on hemodialysis, the as-needed maximum is 5 mg not more than once every 72 hours. Hemodialysis between 24 and 30 hours after a dose contributed negligibly to clearance in the label's study. Do not 'make up' a missed weekend dose after dialysis.
Ketoconazole and ritonavir cap the daily schedule even harder: once-daily Cialis should not exceed 2.5 mg with potent CYP3A4 inhibitors. Mild renal impairment does not need an as-needed cut. Moderate hepatic impairment is a caution; severe hepatic impairment is a place the as-needed label does not support routine 10 mg use.
