Weekend sex and a weekday prostate do not want the same blister. That is the fork I keep drawing on paper when someone asks whether a daily Cialis 5 mg tablet is 'the same thing' as a 10 mg tablet taken on Friday. Same molecule. Same nitrate ban. Two labelled plans, studied and written as separate regimens. Mixing them because the arithmetic looks tidy is how people end up with a peak they never asked for on a Tuesday, or a flat level that never quite arrives.

Ten milligrams before a planned weekend is the as-needed start on the U.S. label for most men. Swallow it prior to anticipated sexual activity. The studies that made the 36-hour claim measured erectile function against placebo out to that mark. A Friday evening tablet can still be doing useful work on Sunday morning. No other tablet on this shelf stretches that far on a single swallow.
Onset is often felt within about 30 minutes. The median time to peak blood level is still 2 hours, so a man who swallows at the restaurant and expects an automatic result at dessert is asking the clock for something the absorption curve does not promise. Food does not have to be cleared first. Unlike sildenafil, tadalafil may be taken without regard to meals. That is one reason a weekend 10 mg is easier to time around a late dinner than a 25 mg sildenafil tablet.
The weekend plan suits infrequent or predictable sex. If intimacy happens two or three times a month, thirty morning tablets are thirty days of drug for a handful of evenings. The 10 mg tablet also leaves room to move: 20 mg if the peak is not enough, 5 mg if flushing or a headache is too much. Those steps stay on the as-needed ladder. They are not a licence to add a 10 mg tablet on top of an ongoing 5 mg daily run.

Prostate symptoms are the one medical reason the daily 5 mg tablet can win even when sex is infrequent. Night-time trips, a weak stream, a bladder that never feels empty: those complaints live in the same smooth muscle the drug relaxes in the penis. The FDA added the BPH indication for once-daily Cialis in 2011, after the as-needed ED tablet (2003) and the daily ED regimen (2008). As-needed 10 mg was not studied or labelled as a BPH treatment. Using a weekend tablet 'for the prostate' is an invention, not a plan.
The label draws a second line that people flatten. Tadalafil 5 mg daily is approved for BPH on its own. Combining it with an alpha-blocker such as tamsulosin specifically to treat BPH is not recommended, because both drugs lower blood pressure and the pair has not been adequately studied for that indication. For ED, a man already stable on an alpha-blocker may still start tadalafil, at the lowest recommended dose, after the prescriber has seen the blood-pressure picture. Those two sentences are easy to swap and dangerous to swap.
If an alpha-blocker is already on the list for urinary symptoms, the conversation is a switch or a supervised overlap, not a second bottle opened at home. I want the urologist who knows the flow studies in that room. The short guide can only name the line.

Missed mornings, stacked tablets, and the nitrate wall
One forgotten morning tablet does not empty the tank. The half-life is long enough that a single miss dips the plateau and a resumed dose rebuilds it. Take the next tablet at the usual hour. Do not double. Miss three or four mornings in a row and you have left the plateau. Expect a few days of daily dosing before the floor is back. Half-following a daily plan - most days, then a gap, then a catch-up - gives you neither the flat level nor a clean 10 mg peak.
Switching plans is simple on paper and should still go through the prescriber. Daily to weekend: stop the morning tablet, then use 10 mg when you need it. Weekend to daily: start the morning dose and stop event dosing. A 10 mg or 20 mg swallow on top of ongoing 5 mg daily is two plans at once. The label's frequency limit is one tadalafil dose per day, not one plan plus a bonus.
Nitrates sit outside every schedule debate. Organic nitrates and nitric oxide donors - nitroglycerin tablets or sprays, isosorbide mononitrate or dinitrate, recreational amyl nitrite 'poppers' - are contraindicated with tadalafil. So is riociguat. The combination can drop blood pressure to a dangerous level. After a tadalafil dose, at least 48 hours should pass before a nitrate is even considered in a life-threatening setting, and then only under close monitoring. Daily 5 mg means drug is present every day. A spray used 'twice a year' does not schedule the chest pain that makes someone reach for it.
Potent CYP3A4 inhibitors such as ketoconazole, itraconazole or ritonavir raise tadalafil exposure. On the as-needed plan the labelled cap is 10 mg no more than once every 72 hours. On the daily plan the cap is 2.5 mg. Creatinine clearance below 30 mL/min, or dialysis: daily use is not recommended; as-needed use is capped at 5 mg no more than once every 72 hours. Those limits are why a weekend 10 mg is not a casual default in a man with a thin eGFR.

How I sort the two orders in clinic
Frequency first, then the prostate, then the cabinet. If sex is twice a week or more, or if the pill-before-sex ritual has become part of the problem, daily 5 mg has the better shape. If sex is a handful of planned evenings a month and the stream is fine, a 10 mg weekend tablet wastes less drug and still covers a long window. If urinary symptoms are the second complaint, daily 5 mg is the labelled tool; the weekend tablet is not a BPH dose.
Side effects can flip a first choice. Back pain and muscle ache show up with tadalafil more than with the shorter PDE5 tablets, likely because the molecule also touches PDE11 in skeletal muscle. On a daily plan the ache can feel continuous. Dropping to 2.5 mg daily, or moving to 10 mg only when needed, are both labelled moves. They are not DIY splits of a 20 mg tablet invented at the kitchen counter.
Cost is real and this shelf does not invent a cash figure. Generic tadalafil exists in both strengths. Thirty morning tablets can cost more or less than eight weekend tablets depending on the pharmacy and the discount in front of you that week. Compare the actual fill in your hand. Do not pick a schedule from a number you saw on a banner. If budget is the only reason someone is skipping morning doses, the weekend plan is the honest one.
New erectile trouble after 50 still wants a cardiovascular look before anyone optimises a blister. The penile arteries are narrow. They often complain before the coronary ones do. Untreated diabetes, a thiazide that could be swapped, or a testosterone that has not been measured: those belong in the same visit as the schedule talk.

Why a morning 5 mg tablet stays in the picture
Five milligrams every morning is a small tablet that lands on yesterday's leftover drug. The mean terminal half-life in healthy people is 17.5 hours. That is long enough that a once-daily dose does not vanish overnight. After several days the rise slows and the level sits in a modest plateau. From then on there is no pill to swallow an hour before sex, and no glance at the clock that announces the evening has been scheduled.
The label for once-daily use says take it at about the same hour, without regard to when sex might happen, and without regard to food. Tadalafil's peak after a single swallow sits anywhere from 30 minutes to 6 hours, with a median of 2 hours. On the daily plan that peak is not the point. The point is the floor that remains between doses.
Couples mention the floor more than they mention milligrams. The tablet stops being a cue. For men whose erection trouble is half blood flow and half the performance countdown, that change is the treatment, not a side benefit. It still needs arousal. A PDE5 inhibitor does not invent desire and does not raise an erection in an empty room.
Daily dosing is also the only tadalafil plan that carries a BPH claim. The 5 mg tablet, taken at the same time each day, is approved for the signs and symptoms of an enlarged prostate and for men who have both ED and those urinary symptoms. The 2.5 mg daily tablet helped erections in the combined study; it did not produce a statistically significant change in the total IPSS. What I write back after the first month
After four weeks I want one sentence about the week, not a score. Did the morning tablet disappear into the background, or did it become a chore on days nothing happened? Did the 10 mg Friday tablet cover the weekend without a second swallow? Did the night-time bathroom trips ease on 5 mg daily, or was that hope talking? Those answers tell me whether to keep the plan, step the as-needed dose, or stop treating a schedule that was never going to fit.
Pick one labelled plan with the prescriber who can see the nitrate list and the creatinine. Revisit it. Plenty of men switch once.
